Follow these steps to complete the application process:

K-12 Education Fireworks Display
Step One:
Fill out the Local Official Authorization Form and upon completion, print and provide to the appropriate local official for signature. It must then be uploaded in Step two.
Step Two:
Fill out the K-12 Education Fireworks Display Application and upload the Local Official Authorization Form from Step One below.
